The mid-level performs all functions for basic delivery of healthcare services. The mid-level will plan, implement, and evaluate care per supervisory agreement with collaborating physician to include in-office patient care performing comprehensive and problem-focused histories and physical examinations, diagnosing and treating illnesses and injuries, ordering and interpreting lab and radiographic studies, patient education, procedural interventions, medication management, and referral facilitation; consult with the patient and members of the healthcare team to provide for acute and ongoing healthcare and referral of the patient; manage the medical plan of care prescribed for the patient based on protocols or guidelines adopted jointly by the mid-level practitioner and the physician; initiate and maintain accurate records, appropriate legal documents and other health and clinical reports; participate with physician in periodic review of the patient’s health records. Mid-level will provide services in accordance with the policies of the medical facility; arrange for or refer patients to needed services that cannot be provided at the clinic; assure that adequate patient health records are maintained and transferred as required when patients are referred; participate in periodic and joint evaluation of services rendered, including but not limited to chart reviews, patient evaluations, and case outcome statistics. Mid-level will adhere to required quality programs; follow all policies and procedures established by WGH and WCHC; follow safe practices for handling toxic substances, infection control, and fire and safety procedures; must have knowledge and skills necessary to provide service to customers of all ages.
