With the supervision of a QMHP, work collaboratively with on-call MHP/QMHP to provide crisis services, counseling and case management services to children, adolescents and adults with mental health issues.
Illustrative Examples of Work:
Participate in crisis response with MHP/QMHP during on call rotation;
Conducts work under the supervision of QMHP/MHP or Crisis Director;
Works collaboratively with other disciplines to arrange services beyond his/her scope of practice;
Assist clients in identifying and engaging various community resources and supports;
Provide culturally competent peer counseling services to individuals experiencing;
Act as an advocate for individuals in crisis and individuals on-scene, ensuring their needs are met and barriers treatment are removed;
Maintain the LCHD warm line during on call rotation;
Provide case management services to clients as directed by supervisor;
Provides client information in accordance with HIPAA guidelines, to other agencies or healthcare providers to coordinate services to identified clients;
Participate in clinical and administrative supervision, case conferences, staff meetings, in-service training, and other staff development activities;
Ability to travel within the state of Illinois.
Education and Experience:
High School Diploma, GED or higher education;
Lived experience as a consumer of mental health or substance abuse services.
Skills, Knowledge, and Abilities:
Requires demonstrated ability to work constructively with clients, other providers and the community
Requires knowledge of community resources for referral purposes
Requires understanding and ability to put into practice the Mental Health Code and Confidentiality Act.
